Adams County Sheriff Shane Rekeweg announced Friday that there has been a third fatality resulting from the April 17 accident involving an Amish buggy and a van on SR 124 near CR 400W.
Rekeweg said Jerry M. Schwartz, 17, who has been hospitalized since the accident, has died as a result of his injuries.
No information regarding the condition of Mary M. Schwartz, 44, who was transported by helicopter to a Fort Wayne hospital, has been released.
The crash occurred just before 8 a.m. when a van driven by Chandler M. Gerber, 21, of Bluffton, was traveling east on SR 124 and failed to see a buggy that was also traveling east.
According to the police report, Gerber stated he was driving approximately 60 mph when he hit the rear of the buggy, which had six passengers.
Also killed in the crash were Enos Schwartz, 3, who died at the scene, and Barbara M. Schwartz, 5, who was pronounced dead shortly after being transported to Adams Memorial Hospital.
The accident remains under investigation, according to the report.
